How do I report harassment at work in writing?

Follow your employer's anti-harassment policy. It names who receives reports. Describe each incident factually: date, place, who, exactly what was said or done. List witnesses and attach any messages or notes, and explicitly request an investigation and protection from retaliation.

Should I keep my own records of harassment?

Yes. Keep contemporaneous notes, written down close in time to each incident, plus copies of messages, and store them somewhere personal, not only on a work device. Contemporaneous records carry real weight in investigations.

What if the harasser is my supervisor?

Policies provide an alternate reporting path for exactly this, typically HR directly or another designated officer. You are not required to report through the person the grievance is about.

What if my employer ignores the harassment grievance?

Document the non-response. Depending on the conduct, you may be able to file a charge with the EEOC or your state's fair employment agency, both have strict filing deadlines measured in months, so don't wait long. An employment attorney can assess the situation; many offer free consultations.
